Rib Fractures in Blunt Thoracic Trauma, New Zealand Management and Outcomes

Health inequities are evident in trauma outcomes across Aotearoa.

The Aotearoa New Zealand RiBZ study examined outcomes for patients with isolated rib fractures across 14 hospitals. While overall pneumonia and mortality rates were comparable across centres, Māori patients experienced a significantly higher rate of 30‑day re‑presentation, even after adjusting for age and injury severity. Older patients were also at increased risk.

Access to evidence‑based interventions was limited and variable nationwide, with low utilisation of regional analgesia and surgical stabilisation of rib fractures, particularly outside tertiary centres.

 

These findings indicate that inequities arise not from acute hospital care alone, but from differential access to resources, follow‑up, and system responsiveness — highlighting clear targets for quality improvement to reduce avoidable disparities in trauma care.
